Medical cards for people from Ukraine

You can apply for a UKR medical card if you arrived from Ukraine less than 1 year ago and you have temporary protection.

You need a personal public service (PPS) number to apply for a medical card. If you do not have a PPS number, go to your local Intreo centre or branch office. Staff will help you to get a PPS number.

Apply for a UKR medical card

To apply for a UKR medical card, you must complete an application form.

The form is available in the following languages:

If you have a GP, ask them to complete and stamp section 3 of the form. If you do not have a GP, leave section 3 blank and we will assign a GP to you and your family.

Email the completed form to pcrs.applications@hse.ie.

Make sure you include copies of all supporting documents.

After 1 year in Ireland

After 9 months, we will send you a letter to complete an assessment to renew your medical card.
