- send you vaccination details
- make sure that we can identify you correctly against your Individual Health Identifier (IHI)
- do public health reporting
PPS number
We need your PPS number to match your vaccine registration to your Individual Health Identifier (IHI). Your IHI is the number that matches you to your existing health records.
This means we can correctly identify you when you come for your vaccine.
Email address
We ask for your email address so we can send you an email verification link. This helps us to verify your email address before registration and to keep the system secure.
Mobile number
We ask for your mobile number so we can send you a 5-digit code before registration. This helps us to verify your mobile number before registration and to keep the system secure.
We also use your mobile number to send you appointment details by text message and to prepare you for vaccination.
Nationality
We ask for your nationality to help us match your vaccination registration to your Individual Health Identifier (IHI). Your IHI is the number that matches you to your existing health records.
This means we can correctly identify you when you come for your vaccine.
Ethnic background
We ask for your ethnic background to help us with public health reporting.
It is OK to answer "Prefer not to say".
GP
We ask for your GP's details so we can update them about your COVID-19 vaccination.
Gender
We ask for your gender as it appears on your passport, medical card or national age card, so we can match your registration to your Individual Health Identifier (IHI).
Your IHI is the number that matches you to your existing health records.
We understand this may not match your current gender or how you self-describe but it is important that we are able to correctly identify you when you come for your vaccine.
How we collect and use personal information
To give you your COVID-19 or flu vaccine, we need to collect and process some personal information about you.
Only necessary data is recorded. We only collect the information we need to identify you, vaccinate you safely, record your vaccination and monitor the vaccination programme.
