Disabilities Home Support Packages

The CHO 6 Service Directory is intended to inform both staff and service users of HSE and Community Services available to them, how to access, how to contact the services, and the physical/ online location of services. United Nations Convention for the Rights of Person with Disabilities (UNCRPD) underpins how Disability Services are delivered and prioritised within the Wicklow area within the available Disability Resources annually. Disability Services support adults and children with ID and/or autism or physical and/or sensory disability living at home. A Home Support Package is to assist an individual in Activities of Daily Living for an individual and only for that individual. A Home Support Carer provides assistance for personal care such as assisting in/out bed, toileting, and showering/washing/dressing.

Public health nurse, health and social care professional

Referral information

Complete an assessment form with the Public Health Nurse or Health and Social Care professional. Fill out all areas with particular attention to the proposed schedule and essential Activities of Daily Living (ADL's). Supporting documents or reports, if available, can be submitted with the application
