Parent or legal guardian - requesting information

If you have received a letter about a child you are a parent or legal guardian to, you can request copies of the documents related to your child.

Request documents

You can request copies of the documents related to your child, by post.

This is the only way to request their documents. You can no longer do this online.

What you will need

To make a request, you will need:

  • your PIN number - you can find this on the letter we sent you
  • to print out and complete the Data Access Request Form - Parent or Legal Guardian (PDF, 325 KB, 3 pages)
  • to photocopy your current and in-date passport, driving licence or public services card - photocopy both sides of your driver’s licence or public services card
  • to photocopy proof that you are the child's legal guardian

Send your completed form, photocopy of your ID and photocopy of your proof of guardianship to:

If you have been told that more than 1 of your children has been impacted by the cyber-attack, you can use the same form to request information about all children.

How long your documents will take

It can take up to 1 month for the documents to be ready from the date that we receive your completed form and ID.

In some cases, it may take up to 2 months longer. We will let you know if this happens and explain why.

When we send you your documents we will include details of how to contact us. This is if you want to speak to us about the information you receive.

Proof that you are a legal guardian

You will be asked for copies of documents that show you are the child's parent or legal guardian.

The documents you need to provide depends on your relationship to the child.

Mother

You will need to provide a copy of one of the following:

  • the child's birth certificate (on which you are named)
  • the court order appointing you as the child's legal guardian
Father who is a legal guardian

If you are married to the child's mother, you need to provide a copy of both of the following:

  • the child's birth certificate (on which you are named)
  • a copy of your certificate of marriage to the child's mother

If you are not married to the child's mother, you need to provide a copy of one of the following:

  • legal guardianship declaration you signed with the child's mother
  • the court order appointing you as the child's legal guardian
Other legal guardian

If you are the child's legal guardian, you will need to provide a copy of one of the following:

  • the court order appointing you as the child's legal guardian
  • other legal or court documents appointing you as the child's legal guardian

If you need to change your postal address

To update a child's postal address you will need:

  • your PIN number - you can find this on the letter we sent you
  • to print out and complete the Request for Rectification/Erasure of Personal Data Form (PDF, 201 KB, 3 pages)
  • to photocopy your current and in-date passport, driving licence or public services card - photocopy both sides of your driver’s licence or public services card
  • to photocopy proof that you are the child's legal guardian
