If you have received a letter from us about the cyber-attack, you can request:
- a call-back from the HSE
- copies of the documents related to you that were illegally accessed and copied
The quickest way to do is by registering online.
If you find it difficult to register online or do not want to, you can do one of the following:
- Ask someone you trust to register online for you.
- Request the documents related to you by post.
- Register by post to get a call back - you can request your documents on the call or get help to register online, if you want.
Request documents by post
If you want access to the documents related to you, you can make a request by post.
To request the documents related to you by post you will need:
- your PIN number - you can find this on the letter we sent you
- to print out and complete the Data Access Request Form (PDF, 1.4 MB, 1 page)
- to photocopy your current and in-date passport, driving licence or public services card - photocopy both sides of your driver’s licence or public services card

When we receive your completed form and ID
It can take up to 1 month for the documents to be ready from the date that we receive your completed form and ID.
In some cases, it may take up to 2 months longer. We will let you know if this happens and explain why.
You will be told when we expect to have the documents ready for you.
Register by post to get a call back
You can register by post. You'll need to complete a form and send us proof of your ID. We will then call you.
On the call you, you can ask us:
- questions you have about your letter
- to request copies of the documents related to you and receive them by post
- to complete your registration so that you can use the online data notification service to request your documents online

To register by post you will need:

- to photocopy your current and in-date passport, driving licence or public services card - photocopy both sides of your driver’s licence or public services card
