You can apply for the Drugs Payment Scheme (DPS) online, by post or by email.
For anyone included in the application you'll need to provide their:
- full name
- date of birth
- sex
- PPS number
- address
- contact details
You will also need your DPS card number, if you have one.

Complete the form and post it to:

Dublin 11
Or email a scanned copy or clear photo of the completed form to pcrs.applications@hse.ie.
Incomplete applications will take longer to process as we may need to contact you for more information.

- get a form from your local local health office
Temporary emergency registration
Your pharmacy may submit a temporary, once-off, emergency registration for you. You will still need to apply for the scheme within 3 months.
The digital version of your card will appear in the HSE Health App.
