Apply for a Drugs Payment Scheme card

You can apply for the Drugs Payment Scheme (DPS) online, by post or by email.

For anyone included in the application you'll need to provide their:

• full name
• date of birth
• sex
• PPS number
• address
• contact details

You will also need your DPS card number, if you have one.

Complete the form and post it to:

Eligibility Unit
PO Box 12966,
Dublin 11

Or email a scanned copy or clear photo of the completed form to pcrs.applications@hse.ie.

• Phone 0818 224 478 to order one over the phone
• get a form from your local local health office

Temporary emergency registration

Your pharmacy may submit a temporary emergency registration for you. You will still need to apply for the scheme within 3 months.
