Dietetic Service Adults
Dietitians are qualified and regulated Health Professionals that assess, diagnose and treat dietary and nutritional problems. They use the most up to date public health and scientific research on food, nutrition, health and disease which they translate into practical guidance to enable people make appropriate lifestyle choices to address nutritional problems. Adult Referral Types (1) Home Enteral Feeding (2) High Risk of Malnutrition (3) Non acute Gastrointestinal Disease (4) Chronic Kidney Disease stages 1,2 and 3a only (5) Hyperlipidaemia/metabolic Syndrome (6) Weight Management (7) Diagnosed nutritional deficiency. Full referral acceptance and non acceptance criteria available on request The Dietetic care of Type 2 Diabetes is provided in the Integrated Care Hub for Chronic Disease (Bray/Clonskeagh) Referrals for either individual care or the DISCOVER DIABETES - Type 2 course are via Healthlink referral by a patient’s GP or Practice Nurse (use drop down option of Integrated Care Hub) Patients can self-refer to DISCOVER DIABETES – Type 2 by registering their interest on www.hse.ie/diabetescourses or by calling HSE live on 1800 700 700. The Dietetic care of patients with Prediabetes is provided in the Integrated Care Hub for Chronic Disease (Bray/Clonskeagh). Diagnosis HbA1c 42-47 mmol/mol (ideally in the last 3 months) and/or Fasting Plasma Glucose 6.1-6.9 mmol/l Referrals are via Healthlink referral by a patient’s GP or Practice Nurse (use drop down option of Integrated Care Hub) Weight Management/Obesity Please consider if the patient is eligible for The Best Health Weight Management programme provided in the Integrated Care Hub for Chronic Disease (Bray/Clonskeagh) using the following criteria: • BMI ≥ 30kg/m2 (BMI ≥27.5kg/m2 for People with South Asian, Chinese, Black African or Caribbean family backgrounds will be considered) and • Two or more obesity co-morbidities present; Type 2 diabetes, hypertension, hyperlipidaemia, obstructive sleep apnoea, polycystic ovarian syndrome, osteoarthritis. Referrals are via Healthlink referral by a patient’s GP or Practice Nurse (use drop down option of Integrated Care Hub) If a patient does not meet the above criteria they can be referred to the dietitian in their local Primary Care/Health Centre where they may be offered a defined number of appointments for Weight Management
