Medical card for children with cancer

Children who have been diagnosed with cancer within the last 5 years can get a medical card.

You don't need to supply income and outgoing details. The application form just requires a medical report from your GP or hospital doctor.

How to apply

  1. Download the application form for children with cancer in English or Irish
  2. Complete the form
  3. Send the completed form to the National Medical Card Unit, PO Box 12629, Dublin 11
