At any time you can request your cervical screening:
- records
- slides for independent review
If you have been diagnosed with cervical cancer and also had cervical screening, you can ask for a personal cervical screening review.
Cervical screening records
You can access your cervical screening records.
You may also access the records of:
- a relative who died
- someone you represent professionally
If you are requesting records on behalf of someone else, you will need to send us proof that you have the right to request this information.
What to include in your request
You must include all of these details in your request:
- name
- address
- date of birth
- contact phone number, if we need to contact you about your request
- ID - a scanned copy or a photocopy of your passport or driver's license
- what records you want - all your CervicalCheck records or specific records

When we get your request
When we get your request we will:
- let you know that we have your request
- contact you if we need any more information
- tell you if we need more time
- process your request when we have all the information we need
- send you your records within 1 month
Slides for independent review
You can request your previous cervical screening slides for independent review.
We will only gather the information when we have your consent to do so.
