Your Service Your Say is how we listen and respond to your feedback about our services.

You can use Your Service Your Say to make a:

  • comment or suggestion
  • compliment or positive feedback
  • formal complaint

Types of feedback

What to include in a formal complaint

Give as much detail as you can when making a formal complaint.

Include:

  • what happened and when
  • who was involved
  • where it happened
  • what your concerns are
  • anything you have done to resolve this matter
  • what you want to happen now
  • any extra information and copies of other relevant documents

What happens next

If you contact us using an online form or email, you'll get an automatic email to tell you that your feedback has been sent to Your Service Your Say.

We'll send your feedback to the service you gave feedback about.

We may phone or email you before we do this if we need more information about your feedback.

If you made a complaint, a complaints officer from the service will email or write to you. They will do this within 5 working days.

If you made a comment or compliment about a service, we usually do not need to contact you. But sometimes a staff member from that service may phone or email you about it.

Making a complaint for someone else

If you're not able to give feedback yourself, ask a relative, carer or advocate to do this for you.
