Living well programme

Living Well is a generic self-management programme. It is a free group programme for adults with long-term health conditions. It supports participants to develop skills which help them to live well with their health condition. These skills are the toolkit for better health. Living Well is delivered by 2 trained leaders over six workshops each lasting 2.5 hours. People taking part in the workshops have different health conditions.

Adults aged 18 years and over with long-term health conditions. Carers of adults with long-term health conditions. Staff with long-term health conditions. Not suitable for people with significant memory or learning difficulties.
