About this service
Community Specialist Palliative Care teams provide specialist care, support and advice to patients and families in the persons place of residence e.g. home, nursing home. The community specialist palliative care team work closely with the persons General Practitioner and other health care staff to ensure the patient receives timely and integrated care in the community.

All patients with a specialist palliative care need and their families and carers.
Who needs to refer you
Referrals are accepted from the Hospital Consultants or General Practitioners or by a member of the Specialist Palliative Care team. A formal letter may be sent to one of the Consultants in Palliative Medicine.
