High-risk specialised care pathway pregnant women with gestational diabetes, type 1 or 2 diabetes, maturity onset diabetes of the young (MODY) and latent autoimmune diabetes in adults (LADA)

Referral information
Referral letter to include name, date of birth, address, LMP, clinical diagnosis, medical history, surgical history, list of co-existing medical conditions, list of comorbidities, list of any past/current mental health issues, list of other medical or surgical services the patient is attending, list of current medications.
Internal directions
Main clinic UMHL - proceed through the front door, take the first right through double doors, proceed past the shop, take the stairs going down and continue straight at the end of the stairs and straight down the corridor to the end.
