Admission may be considered: to manage complex symptoms to promote comfort, autonomy and rehabilitation management; rehabilitation to continue to live independently; respite care to enable carers to have a break; care as illness progresses towards the end of life.

Who needs to refer you
Referrals are accepted from the Hospital Consultants or General Practitioners or by a member of the Specialist Palliative Care team. A formal letter may be sent to one of the Consultants in Palliative Medicine.
